結構訳しにくい表現です。 in the end (最終的には、終いには)が一番近い。何かをネガティブなことが続けて起きたということを強調するなら、 “on top of that” や “to make matters worse”が使えます。なお、少し意味はズレますが “to rub salt into the wound/to add insult to injury” (傷口に塩を塗る) というイディオムも、悪い状況がさらに悪くなる様子を表せます。
